Set Goals and Make an Action Plan That Works for You

  1. Think about what matters most to you, what are your priorities for health?
    Your health care provider can help you sort out your priorities.
  2. Set goals and create an action plan that will help you to address your priorities for health and brain health.
    Your health care provider can help to set goals that are realistic and focused.
  3. Plan the actions that will help you meet your goals to improve your heart and brain health.
    Your health provider, family and friends may have resources to share to help you on your health journey. You can make this change!

    My Heart and Brain Healthy Plan belongs to you and it’s your commitment to try to change a behaviour on your terms and only when you are ready! Complete My Heart and Brain Healthy Plan each time you begin to work on a goal and ask your health care provider, family member, or friend to check in with you about your commitment and progress towards meeting this goal.
